It is quite rare in patients who have a structurally normal heart or have never had an ablation but in the post-ablation setting it is important to differentiate atrial fibrillation recurrence vs atrial flutter vs atrial tachycardia as the repeat ablation procedure strategies will be different. This is called ‘atrial tachycardia’ and can be quite hard to differentiate from atrial fibrillation or atrial flutter. Just to complicate matters further, a singular short circuit can occur in different regions of the heart to the area where atrial flutter occurs. Pathway of the AFFU-AW study- currently recruiting at St Bartholomew's Hospital. Have your symptoms come back because you’re having a recurrence of the original arrhythmia or is this a new arrhythmia you’ve developed? Having had one arrhythmia puts you at higher risk of developing the other, so this is not an uncommon situation! This is one problem that the AFFU-AW study is exploring whether putting the power of ECG diagnostics in the hands of the patient improves the time to detection of recurrence and importantly, if it can help to identify the cause of the symptoms. Your symptoms and the A&E doctors' treatments could be exactly the same, the only way to know whether you had an episode of atrial fibrillation or atrial flutter, is if you can show them the ECG.Įspecially if you’ve already had an ablation treatment for either arrhythmia. Atrial flutter can look like a 'saw-tooth' pattern due to the cyclical short circuit that consistently drives the heart beat.Image adapted from here. This is why your electrophysiologist emphasises the importance of getting a copy of your ECG if you ever attend A&E.- Atrial fibrillation can look like a noisy line on the ECG with irregular spikes. Unfortunately, your risk of developing future atrial fibrillation is higher the two arrhythmias share risk factors and 30% of patients with atrial flutter will go on to develop atrial fibrillation in the next few years. But (there’s always a but!) just because you’ve been cured of atrial flutter doesn’t mean you won’t get any future palpitations. In fact, many skilled electrophysiologists quote cure rates of >90% when treating atrial flutter with ablation. ![]() This means the ablation procedure is more straightforward and there is a high likelihood that the ablation could be curative. Atrial fibrillation is maintained by chaotic, unpredictable electrical activity, usually in the top left chamber of the heart. Atrial flutter is a short circuit in the top right chamber of the heart. ![]() The pattern on your ECG is typical and importantly, the location of the short circuitry in the atrium is fixed and singular. So although the electrical activity is abnormal, it is predictable. Alternatively, atrial flutter occurs because of a single ‘short circuit’ that overdrives the normal heartbeat. Especially in long-standing AF, the triggers can be all over the atrium chambers and it can be difficult to know where to treat. This is what can make it challenging to treat with ablation. It’s unpredictable and changes beat-to-beat. ![]() Both can be managed with rate control tablets (like bisoprolol or verapamil) or rhythm control (like electrical cardioversion).Ītrial Fibrillation originates from chaotic electrical activity in the atrium that overdrives the normal heartbeat. Both can increase your risk of stroke and may warrant blood thinners. And it’s important as a patient to know this, so you can receive the right treatments.įrom the outside, the similarities are clear- both can cause debilitating palpitations, shortness of breath, and lightheadedness. But from an electrophysiologist’s (a cardiologist specialising in heart rhythm disorders) perspective- there are some very important differences. I used the term 'little brother' above in jest- Atrial flutter can be equally debilitating as atrial fibrillation and also carries similar risks.īoth, atrial flutter and atrial fibrillation are abnormal, often fast, heart rhythm disorders that have a lot in common.

![]() I attended Wayland High School, which is located around forty minutes outside of Boston and is surrounded by more trees and boredom than you can ever imagine. This is the true benefit of living in or around Boston and playing Fallout 4: your lines between reality and fiction are blurred slightly enough to where the game itself is far more immersive. The next day, when I rode through Park Street Station on my way to Downtown Crossing, I actually had a bit of trouble ignoring the idea that gangsters with submachine guns might just poke their heads around the corner. After all, this is an area that is particularly near and dear to my heart (in fact, it’s right next to where I saw that insane ADR1FT demonstration on a movie screen at PAX East 2015), so seeing this awesome area in such a decayed state is harrowing. Everything from the small structure containing Park Street’s escalators to the fact that the center platform of this medium-sized T stop is modeled perfectly made for a chilling experience. What you’re tasked with doing from a gameplay perspective isn’t necessarily important in this discussion, as the real star of the show here is the way that the Commonwealth’s architecture models its real-world equivalent so closely. Without spoiling any of the story content in this particular mission, the goal was simple: break into Vault 114, which happens to be located in Park Street Station, which is located around the Boston Common in both real life and in Fallout 4. Never was this more evident than in an early mission that took me through a T station that I’ve passed through hundreds of times. Some of the more minute details that Bethesda has put into the Commonwealth are downright staggering, from the way that MIT’s dome is shaped to the fact that walking along the Freedom Trail actually feels like walking along the Freedom Trail, vibe and all. ![]() Yes, Fallout 4 contains a whole host of things that are not in the current version of Boston, but make no mistake, this definitely feels like an alternate version of the Greater Boston area. When a significant portion of your life is spent in or around a city like Boston, and all of its oddities and charm (namely cigarette-eating homeless men, angry pedestrians and random stray dogs growling outside of sandwich shops), you’re going to know right away whether or not the spirit of your city is captured in a piece of media. I currently live around twenty minutes outside of Boston, with a trek into the city using involving a short bus ride and then a couple stops along the T’s Red Line. Sure, there’s The Last of Us, the aforementioned Tony Hawk’s Underground 2 and Assassin’s Creed III, but no game has truly captured the feeling of exploring this wonderful city and its suburbs, until now. If you live in or around the Boston area, you don’t have the widest selection of titles to choose from to get your homely fix. Gamers who live in Seattle got the chance to experience their hometown in Infamous Second Son, residents of Barcelona got to shred up their sidewalks in Tony Hawk’s Underground 2 and players living in San Francisco witnessed their most notable landmark get destroyed in Call of Duty: Advanced Warfare. We’ve seen tons of games set in real-world locations, but every once in a while a title comes along that has the one location that you hold near and dear to your heart: your home. ![]() This can frequently be the reason a dash cam stops working as less than great connections shake loose or cables even drop out altogether. It’s worth checking this again once the setup has had time to bed in, especially if you own an older vehicle that’s prone to lots of vibration. Most models come with a bundled cord for doing this, though it's worth making sure it comes with a longer one (at least four meters, ideally) if you're planning to pipe it around your windscreen and underneath seats. The most basic way to power your dash cam is to plug it into your car's cigarette lighter/auxiliary port. Plug-in or hard-wired?ĭash cams are third-party accessories, which means you'll have cables to deal with – and this is the most complex part of the installation. Some tape, a wipe-off marker and, ideally, a second pair of eyes can all help get the camera positioned correctly before you fasten it into place. You might get lucky and position it right first time, but if you’re working with a sticky pad-type mount it’s best to practice a couple of times first. Every vehicle is different, so this requires some careful thought and experimentation. Some models come with spare sticky pads that allow you to do this, but it’s less appealing than the suction mount route.Įither way, the main objective is to position the dash cam so it aligns with the horizon. The downside of this mount design is that you need to get the position of the dash cam right first time, or be prepared to pull it off and start again. This bonds in semi-permanent fashion to your windscreen, dash or similar location. The other type of mount frequently revolves around a bracket with a sticky self-adhesive pad on it. (Image credit: xfilephotos / Shutterstock) Conversely, low down near where the windscreen meets the dashboard is another dependable vantage point. ![]() Just under the rear-view mirror is a standard option. If you’re installing a front-facing dash cam you’ll want an unobstructed view of the road ahead from a centrally-located vantage point. This is a straightforward step in the process but requires some forethought. A second pair of hands is always a bonus, too. A light is also handy if you’re working inside the car or poking around under the dash. Grab a screwdriver and some tape for holding things in place. You’ll need to allocate a little time for the installation, but the tools needed should be minimal. Other front-and-rear bundles have a rear-mounted camera as part of the package. Consider this option if you think interior security is a must-have. Some models, like the Nexar Pro, come with an additional interior camera for added security. You might have more than one dash cam to install, too. The Nexar Pro (above) has two cameras, so requires a little more planning in terms of positioning.
